Gender dysphoria and homosexuality: Clinical, conceptual, and ethical analysis of their diagnostic distinction in the contemporary context

The depathologization of homosexuality in psychiatric diagnostic manuals marked a historic advance in the understanding of human sexuality, recognizing homosexual orientation as a natural variation. In contrast, gender dysphoria remains included in current diagnostic systems, which has generated ongoing clinical, ethical, and social debate. This article outlines the fundamental differences between both categories, arguing that gender dysphoria cannot be removed from the clinical domain due to its association with real, structural, and persistent psychological suffering. From a psychodynamic and clinical perspective, it analyzes how a self-perceived identity in dissonance with the biological body produces an internal fragmentation of the self, leading to symbolic dependence on the environment and a compulsive need for external validation. This condition not only has clinical implications but also extends into demands for legal and social recognition through the attribution of rights that contradict biological and objective reality. The article further contends that the application of affirmative interventions without prior clinical exploration neutralizes the role of psychotherapy, hinders the understanding of the etiology of distress, and reinforces disorganizing defensive mechanisms. Authors such as McHugh, Marchiano, and Evans caution that automatic validation may constitute a form of clinical abandonment. It is concluded that ethical and scientific intervention requires upholding the principle of reality, promoting the integration of the self, and preventing clinical practice from being replaced by ideological positions.
